It is your personal responsibility to be knowledgeable of the details of your health care plan, including covered services, copay amounts, and deductible amounts. If you have questions regarding the specifics of your health insurance plan, please contact your health insurance carrier directly.
Your copay and/or any fees associated with non-covered services will be collected at the time of service.

Please notify the front office if you will be late for your scheduled appointment. If you are more than 15 minutes late, you may have to reschedule your appointment. This is important in our efforts to reduce wait times for our patients.
If you have not seen one of our physicians in the last 12 months, refill requests cannot be completed. Some medications may require an office visit for safety monitoring prior to a refill. Refill requests will be submitted within 2 business days.
When scheduling your first visit, you will be asked to pay a $40 new patient fee in order to secure your appointment. This fee will go towards your copay or the cost of your visit. If you do not cancel your appointment within 24 hours this fee is non-refundable.
